In the city where he works, drawing blood for any purpose must be done by trained and licensed medical personnel. Yet despite all this, Sam routinely pierces veins and withdraws their contents. He has never been in any trouble for doing this; in fact, he is paid better the more he does it.

Answer
Sam works in a mine.A vein within an animal's body conveys blood to the heart. A vein is also the name given to a rich source of ore or coal.
Sam would be paid quite well the more of the vein's contents he could procure.
