The title refers to the early days, when doctors were physicians. The risky and messy business of surgery was carried out by barbers, hence the red and white barber's pole, in memory of the blood-soaked bandages. In many countries the surgeon is still given the honorable title of Mister, in memory of the earlier practitioners.
The gun in the clues, refers to the staple gun, which modern surgeons use in place of many of the old fashioned sutures.
I take your money, is obvious, and don't yell that you have free medical care or insurance. The money paid to doctors comes from insurance companies or the government. Either way, whether from premiums, or taxes, it is your money.
