What defines a question as "stupid"?

I think the definition of "stupid" would define a question as "stupid."

Stupid:
1. lacking in common sense or intelligence
2. trivial, silly, or childish

So questions that are lacking in common sense or intelligence; are trivial, silly, or childish; are stupid questions.

"How do I eat a M&M?"

"Now that's just a STUPID question! Eat it like you would anything else. First you peel it, and then you eat the goodies on the inside of it! SHEESH!"

But neither of those are STUPID questions. The first is not a stupid question, because what if somebody has never seen an M&M and their friends told them they arent supposed to eat the coloured stuff?
And "Fred, what is your name?" is not really a stupid question, because obviously the person asking already knows the answer. That's more like a rhetorical question.

I was working at a pizza place a while ago. It has the pizza smell, people eating pizza, a big sign saying New York Pizza, and a big menu.

This girl comes in and asks me, "Do you guys have pizza?"

There are only three explanations for such an occurrence that I can think up. One being that Jill is mentally retarded, thus this cannot qualify as a stupid question to me since she has no choice over it. Two being that Jill is mocking somebody, thus this cannot qualify as a stupid question since it's a mockery and not really a question. Three being that Jill is tutoring some kid and trying to be funny, and again this can't register with me as a stupid question.

Pretty much the same for Cam's girl's question. She was either joking or mentally retarded. Either way, not a stupid question.

1. She's joking
2. She's stupid
3. She's mocking you
4. She accidentally picked the wrong word.

All do not make it a stupid question. You eliminated 1 and 2. So it is either 3 or 4.

I think a stupid question can only be defined based on the context in which it was asked.

In Cam's example, that was indeed a stupid question.
Going into a library and asking if they have books is also a stupid question.

However, asking a bookstore if they carry children's books is not a stupid question, because it could be a specialty bookstore.

I'm sure I'd be able to think of better examples, but I'm too tired at the moment.
