He did not fall out of the train.He took the bandages off after the 30 minutes inside the tunnel.

Actually there is a similiar one to this where he is in a SMOKING cabin. He takes a peek and at first thinks he's still blind, but when the guy sees the lighted cigerattes (dunno how to spell it!) he realizes the operation worked.
