It could also be that john and frank are little boys and are having a fake duel with guns that shoot foam darts. therefor if they shot eachother in the head neither would die.
It said they fought "a duel". That is singular, so I thought that ruled out their fighting separate duels.
I suppose if you read it as "each fought a duel" then it makes sense.

Either they were dueling or they weren't! I just wasn't happy with the answer, sorry.

If they weren't dueling each other, they wouldn't have fought "a" duel. They would have fought 2 duels.

answer didnt make sense to me
I, like quite a few people, thought they used water guns! 
The singular "a duel" bothered me, too. Also thought it was paintball - what a mess!
The first sentence ought to be:
Two men, John and Frank, dueled with guns.Then it will be correct.

I thought they were using paintball guns!
They could've been having a duel in a bathroom on a ship (aka 'head'), and each of them happened to shoot the other non-lethally, in the leg or something. I wouldn't be surprised if their aim was off in a cramped space like that.
I thought it was a water gun. Good one. 
Easy but good. This is sorta like that teaser called "Checkers".
hmm, it says they fought "a duel" not duels as from the answer there is more than one, yet the question suggests it was a singular duel.
