It could also be that john and frank are little boys and are having a fake duel with guns that shoot foam darts. therefor if they shot eachother in the head neither would die.
It said they fought "a duel". That is singular, so I thought that ruled out their fighting separate duels.
I suppose if you read it as "each fought a duel" then it makes sense.

My first thought was Paintball!
If they weren't dueling each other, they wouldn't have fought "a" duel. They would have fought 2 duels.

The singular "a duel" bothered me, too. Also thought it was paintball - what a mess!

The first sentence ought to be:
Two men, John and Frank, dueled with guns.Then it will be correct.

They could've been having a duel in a bathroom on a ship (aka 'head'), and each of them happened to shoot the other non-lethally, in the leg or something. I wouldn't be surprised if their aim was off in a cramped space like that.

hmm, it says they fought "a duel" not duels as from the answer there is more than one, yet the question suggests it was a singular duel.
