I got the shot in the foot part, but where does the head, at the top, come into this?
It was there merely to add context. To have the foot there without the head would have looked funny and the 'himself' was necesary for obvious reasons. Thanks for stopping by ;)
While an injection (noun) is referred to as a shot (noun) it doesn't seem to work with shot (verb) himself. You get injection himself in the foot. I thought of the 'shot himself in the foot phrase' but then rejected it because of this and the aforementioned head.
