If you take the sixteenth and the second and combine them,

Next, not a noun like above,
But spelled the same, no mistake.
In meaning to conduct or to guide,
Also, to bring or to take.
Last of all you shall find
A homophone to the first one,
But it is the same as the second,
Only already done.
Now using the clues tell me,

Lead, the element, has a symbol, Pb. The sixteenth letter of the alphabet is P, and the second, b. Combine the two to get Pb, lead.
Also, at one time, lead was used in certain color paints, but it was eventually discovered that lead in paint was hazardous, so it is no longer used for this purpose.
Lead, the verb, means to conduct, to guide, to bring, or to take. It is spelled the same as lead (Pb), but is pronounced differently.
Led is pronounced like lead (Pb), but is past tense of lead (the verb), and therefore is lead (the verb) already done.
